Abstract Introduction TCF21 is a member of the basic helix-loop-helix (bHLH) transcriptor factor family, being critical for embryogenesis heart. It regulates epicardium-derived cells differentiation into smooth muscle (SMC) and fibroblast lineages. The biological roles in epicardial fate determination progression atherosclerosis remains controversial issue. Purpose Investigate impact rs12190287 G>C variant on prognosis coronary artery disease (CAD) cohort. Methods A prospective study was performed with 1,713 CAD patients (mean age 53.3±7.8; 78.7% male) surveyed terms MACE occurrence an extended follow-up 5.0±4.3. genotyped analysed using dominant model (GC+CC) and, subsequently, compared wild-type GG to evaluate survival probability by Kaplan-Meier. Cox regression analysis all risk factors genetic models assess independent variables associated patients. Results wild genotype present 9.5% population, GC 43.2% CC accounted 47.3% GC+CC showed worse throughout period. After multivariate analysis, this remained equation as HR 1.41 (p=0.033) together multivessel disease, physical inactivity, chronic kidney (CKD) diabetes.
